Students who would like to participate in any of our sports teams will need to have an up-to-date sports physical on file with Coach Alonso.  The sport physical form can be obtained from school, or downloaded from the link below, and taken to the student's doctor or a clinic to be completed.  Sports physicals are only good for one year from the date of the exam, and need to be completed on the form the school provides for students to participate.  Sports physicals do not guarantee a roster spot on any of our teams.

• Healthy Solutions does accept appointments at their facility in West Baltimore (5115 Liberty Heights Ave, Baltimore, MD 21207) and you can contact them directly at 410-466-8686. 

• Healthcare Clinic at Walgreens located at 900 N. Washington Street

• CVS & Walmart: ones that have clinics can provide Sports Physicals for around $50

• Lillian Wald Nursing Center: provides services for the uninsured for free.  Located at 901 N. Broadway.  Hours are 9:00am-1:00pm Monday-Friday
